What are the grants?
The QOL grants will
provide families with a wide variety of supports related to
their medically fragile child with lifetime
physical special needs.
Our organization
(unlike many) is not disease-specific and has been
lovingly created to support and assist families who are
caring for their
physically disabled child - regardless of
what the underlying disease or disability is.
Grants will
be made by providing the necessary tools (product or
service, NOT cash) to a parent, with the
specific goal of helping the parent to be better
able to care for their child, thereby
enhancing the quality of life for the child and his/her
family at the same time.
How does the grant
work?
If a grant is approved by our Medical Advisory
Committee for your child, the grant will provide you
with the product/service requested for your child,
subject to documentation requirements. The grant funds
are not paid to you or the child outright - our
organization works with partner providers to pay for the
approved item in its entirety, and then sends the item
to you.
How do I apply for a
grant for my child?
First, review the information contained on this page
to make sure you understand what the grant is and how it
works. Then, review our pages on
minimum criteria,
funding cycles,
important information,
grant amounts, exclusions
and
application process. If you meet our
organization's criteria and have all the documentation
items required, then you can apply to be pre-qualified
online by clicking here:
PRE-QUALIFICATION QUESTIONNAIRE.
These questionnaires will ONLY be accepted by
electronic transmission,
and will not be accepted by phone, fax or mail.
What happens next?
The pre-qualification questionnaire will ask some
automated pre-screening questions. If you pass the
pre-screening questions, you will be offered the full
online grant application.
How long does it take
to get a pre-qualification decision?
Our goal is to process these pre-qualification
questionnaires within 14 days of receipt, depending on
number submitted and available volunteer staff.
May I submit more than
1 pre-qualification questionnaire for my child?
Only 1
request per recipient per 12 month period will be
accepted for consideration.
What if I do not meet
the minimum criteria for this grant program?
As much as we would like to assist all children with special
needs, our organization's mission statement and programs are
limited to special-needs children living in the family home,
under the age of 18, and with life-time physical disabilities.

minimum criteria, or you are looking for a cash grant, perhaps

MONTHS BEFORE YOU GET A FINAL DECISION ONCE YOU BEGIN THE
PROCESS OF BEING PRE-QUALIFIED, BEING INVITED TO SUBMIT A FULL
APPLICATION, APPLICATION FILING DEADLINES FOR OUR (4) FUNDING
CYCLES AND THE REVIEW AND DECISION PROCESS. |
